CDT Exam: Application & Registration

Please make sure to view and complete the following requirements.

They will need to be uploaded into the form and submitted to the Education and Certification Committee.

• Employment Resume/History (download PDF)
• Please read the Letters of Reference Formatting Page to ensure that your 3 Letters of Reference are in the correct format before uploading (download PDF).
• You will also be required to upload a personal Blank Client Contract Form.

You may begin your CDT Application and return to it as needed prior or submission. When you have completed requirements—please ensure that all documents are uploaded to prevent delays in your application being processed—submit and continue to checkout and payment.
